Changing things up , and training around an injury. That was on today’s tasks .. Little twist on S&S today for Colin and Hope. She is dealing with a fractured bone in her foot so she is taking extra caution and going much lighter. Also performing her get ups - bottoms up
I have been doing heavy get ups for awhile now and I wanted to get a different response from my TGU so I went with the barbell TGU today. 40kg get ups
The training was laid out like this for Colin

In my previous post, I was talking about golfers elbow and where it could be derived from. I did not have enough space to start talking about tennis elbow so here is the second part to the elbow tendonitis post.⁣
.⁣
Tennis elbow is a little different; when you are experiencing pain on the lateral side of the elbow it is usually caused by overuse of 2 muscles brachioradialis, and extensor carpi radialis. Both these muscles are well known to compensate for the bicep as soon as you pronate the forearm/wrist/hand. Which means the bicep shuts off causing the brachioradialis, and extensor carpi radialis to take over for a much larger muscle which increases the inflammation of the much smaller muscles.⁣
.⁣
When we break this down the brachioradialis even though it inserts on the anterior portion of the forearm the muscle is part of the superficial back arm line which is also connected to the extensor carpi radialis. The function of the brachioradialis is to flex the forearm but also it helps with pronation and supination of the forearm as well. So when you pronate the forearm for any reason the tight brachioradialis creates a larger amount of tension not just on itself but also on the extensor carpi radialis increasing the activation of the muscles making them becomes the primary mover of any type of flexion in the elbow, while in a pronated position.⁣

Later this week I will be posting some exercises you can perform to help reduce or eliminate the pain you are experiencing. They also will be in 2 parts, so look out for "fixing golfers elbow" this week.⁣

**Patellar Tendinopathy Isometrics**


Here are a few exercises to try if you or a patient are experiencing patellar tendinopathy. Formerly known as "tendonitis," tendinopathy is when there is pain and dysfunction of the patellar tendon.

Isometric exercises tend to have analgesic (pain-relieving) effects, and also stimulate tenocytes, which are the building blocks of tendons. Therefore, isometrics are an excellent choice for this condition!

The exercises are as follows: •

> 1/2 Kneeling Rear-Foot Propped Isometrics •
> Long Arc Quad Isometric with Band

No exciting picture from PT other than my reward for working hard. Yes, being in the @gamereadyofficial compression/icing sleeve is a reward. When I started PT on the 11th my knee could only bend to 32°. Today, I was able to bend my knee to 83°. Tomorrow is 4 weeks post-op and because I'm progressing well and my quad is firing, my brace was unlocked to 70°. PT works. Hard work pays off. Anyone who’s had biceps tendon pain will know that it can make overhead movements painful. However, the key 🔑 is to find pain free or less provocative ways to load the shoulder/bicep region.

🐱 There’s more than one way to skin a cat. The following exercises are just a few options to consider:

1️⃣ Half kneeling wall roll (one of the only good uses for a foam roller 😉): the half kneeling position assists with trunk/shoulder position and applying pressure into the roller can often make the shoulder less apprehensive to overhead positions.
2️⃣ Prone 90/90 OH punch: this one is deceptively challenging and doesn’t take much weight to be effective!
3️⃣ Resisted ER band punch’s: this helps to introduce shoulder stability whilst limiting biceps aggravation.

✅ No matter the exercises you choose, the focus should be on strengthening the shoulder/scapula region and improving overhead movement capacity as able.

💪🏼 If you’re managing the exercises above (or similar) then you can begin to load the shoulder further...my favourites to introduce at this stage is either the landline press or rack scrape (see my earlier posts on pain free pressing)

Today’s wellness Wednesday topic: pediatric movement! I’ve always been fascinated with how easily young children move, especially when compared to how much adults (think teens and older) move. Pictured is my beautiful niece, Surrey. To be fair, she is a dancer that works on her flexibility, but look at how well her spine, hips and shoulders move!! A very small percentage of my adult patients can do any of these poses. ⁣

I’ve mentioned before that I use a functional movement scale to assess and reassess patients, and 3 of the 4 pictures are movements I ask patients to perform. Children remain more flexible because they never. Stop. Moving. Lol but all jokes aside, they are constantly challenging their muscles and joints in ways that adults don’t. ⁣

As we age and adopt daily postures, some muscles elongate while their partners shorten. Our joints also gradually lose their ability to reverse positions or maximize their range of motion. That being said, it is possible to improve joint mobility and correct muscular imbalances but it takes dedication and consistency. If you look past individual muscles you’ll instead see how zones of muscles with similar function that connect and cooperate during movement. You have a number of bands that run through you body.

Some that are particular important to running are your oblique slings, which cross over your pelvis in the front and back like an “X”.
.
Lately, I’ve been playing around with drill that work to take your body through a shortened and lengthened sling position under load, and stumbled on this.
.
As you can see, it’s very dynamic and requires a lot of stability. The best part is that it’s very analogous to running, which is always what you want with an exercise: specificity.

Got shoulders? Here is a simple Activation Exercise that will help mobilize and stabilize the shoulders.
Push/Pull-Grasp your hands together and practice pulling them apart for a few reps and then pushing them together for a few reps. You should feel your shoulders begin to work. Next, Push and Pull the hands against one another as you lift and lower your arms in front and behind your body. Practice a few reps frequently throughout the day and watch your shoulder mobility and stability improve! The Push/Pull is great to add to your PreHab routine, especially for athletes who throw or use the upper body for power and accuracy. Plus, this PreHab exercise can be done anywhere!

One of the best ways to improve your hip mobility is by doing the hip 90/90 with principles from @functionalrangeconditioning . Using PAILs and RAILs (progressive and regressive angular isometric loading) will improve your active control over the tissue to create lasting change that will stand the test of time (with continuous inputs of course). .

1️⃣ Get into the 90/90 position. If you struggle with that position put a yoga block under your knee. To work on the front leg keep your torso upright and lean forward as a unit, trying to get your belly button to your knee. A stretching feeling should be felt in the glute region. Hold it for 1-2 minutes. .

2️⃣ Start by ramping a contraction starting at the outside of your ankle pushing down on the ground at 20% effort, moving to your shin at 40% then outside of the knee at 60%, outside of the thigh at 80% and finally your strongest safest contraction holding that for 10sec. .

3️⃣ Now start to use your core and the inside thigh muscles to pull yourself deeper into the stretch holding for 10sec.

4️⃣Hold new stretch position for 30 seconds and repeat. .

5️⃣To do the back side hip, reach back to touch your foot and hip and repeat the above parameters except pushing down from the inside of your leg. .

MY EXPERIENCE IN AN INPATIENT FACILITY:

I began volunteering at Casa Colina Rehab Hospital while I was still in college. I volunteer/observe in the Transitional Living Center which treats patients with spinal chord injuries (SCIs), traumatic brain injuries (TBIs), and/or other trauma. Patients range from adolescents to adults and some come for day treatment while others live in the facility.

This setting has really peaked my interest because many of the patients are learning to adapt to their new lives post-injury. For some that means learning to walk again while for others it’s learning to adapt daily activities to fit their new lifestyles.
.
Quick Highlights:
• Patients are treated more long-term here, so PTs typically get to see them through most of their treatment
• This facility has PT, OT, and speech all in one place, so I‘ve been able to observe how other forms of therapy play a role in a patient’s overall rehab
• I’ve been able to observe neuropsych therapy groups to learn about the executive functioning errors that impact patients with TBIs
• I have observed patients using the Ekso Bionic Suit to walk

Overall, this is truly a wonderful facility and has really interested me in potentially working with patients like these one day!
